Title: Inpatient Care Management Transformation Analyst
Duration: 18 months
Location: Akron, Ohio

CTG is seeking experienced IP Care Management Transformation Analysts to join our growing Transformation Practice to support client redesign initiatives.

Required skills:
• 5+ years’ experience implementing and supporting inpatient care management programs is required – preferably within an Epic EHR environment.
• 5+ years previous experience conducting clinical and operational assessments, workflow redesign, coaching and mentoring is required.
• 5+ years of managing process improvement and change initiatives required
• Demonstrated success driving progress toward desired transformational change and performance improvement, while meeting savings goals set for healthcare organizations
• Solid understanding of health care industry and a comprehensive knowledge related to quality best practices such as NCQA, Population Health and regulatory reporting
• Strong understanding of quality measurements, PDSA cycles, and implementing effective operational changes is required.
• Epic Clinical Case Management certification highly preferred.
• Exhibits strong group facilitation skills and ability to interact with diverse teams
• Excellent verbal and written communication skills coupled with an ability to create and present effective and persuasive public presentations
• Ability to work in a changing, fast-paced, collaborative environment, must be self-directed
• Ability to work well under pressure, manage multiple assignments simultaneously while maintaining quality standards and meeting assigned deadlines
• Ability to juggle competing priorities with competing priorities with proactive problem solving skills
• Strong in the ability to support others operational success
• Maintain appropriate confidentiality in working with stakeholders
• Strong emotional intelligence and adept at relationship building
• Proficiency with MS Word, Excel, PowerPoint, Microsoft Outlook, and willingness to adapt to new software
Past experience with the following is required:
• Regularly facilitate efficient, effective quality improvement meetings with healthcare organizations to monitor, present, and discuss progress on the transformation action plan and achievement in milestones
• Develop strategies based on assessments and performance analysis, for improvement that includes specific outcomes and metrics to monitor progress to a goal and make recommendations for improvement
• Design transformation action plans and implement appropriate performance improvement initiatives designed to assist our clients in achieving contractually required transformation milestones
• Monitor and review progress of milestone achievement
• Using data to analyze key cost, utilization and quality data and interpret results to assess the performance toward goals
• Using data to analyze trends and work with stakeholders to agree on and implement proactive strategies to address issues, and measure impact using a PDSA rapid cycle improvement approach; including external healthcare data
• Assist clients in creating work flows to optimize care delivery, introduce best practices improvements, and evaluate outcomes using rapid cycle improvements PDSA methods to reach mutual goals
• Educate and utilize technology tools to support Transformation via Health IT Tools
• Integrate technology tools into operational workflows
• Work with clients to align and integrate care management programs and population health services to support care management efforts and add value to the patient care experience
